Early results from a phase 4 trial reported that Natesto significantly increased median AM T levels without affecting median FSH, LH and semen parameters at 6 months follow-up. Given that hypogonadism has been defined more so based on adults, in children and adolescents, a distinction should be made. If not occurring together, in hypogonadism as hypoandrogenism (Leydig cell dysfunction), then other testicular cell populations, germ cell and/or Sertoli cells, should be considered for possible dysfunction. Therefore, the assessment of Sertoli cells is essential to diagnose hypogonadism in a prepubertal population. There is also increasing evidence that anti-Mullerian and inhibin B levels can improve the sensitivity and result in earlier diagnosis which ultimately allows for treatment to start at a younger age (10).

Although testosterone therapy helps with hypogonadism, whether it helps in the reversal or slowing of normal effects of aging and lower testosterone levels is unclear. Several published research articles have conflicting results and more research is needed to make a final determination about the use of testosterone for healthy aging males. Testosterone preparations can reduce a man’s levels of follicle-stimulating hormone (FSH), which triggers sperm production. Men who are hoping to become a father need to carefully consider the risks testosterone replacement therapy may present to fertility.
It is important for all males considering testosterone replacement therapy to speak to their healthcare providers and undergo prostate screening before starting this therapy. However, those assigned female at birth also have testosterone in their bodies. Research shows that normal testosterone levels in females are vital to maintaining libido, cognitive function, lean muscle repair and growth, and bone strength.
